SMS Terms and Conditions
Program. SD Weight Loss Center sends appointment reminders and confirmations, medication refill reminders, intake paperwork links, and follow-up care instructions by text message to patients who have opted in.
Consent. By opting in you agree to receive text messages from SD Weight Loss Center at the mobile number you provided. Consent is not a condition of receiving care or of any purchase. You may decline text messages and still schedule and receive treatment.
Message frequency. Message frequency varies, typically 2 to 6 messages per month, depending on your appointments and treatment plan.
Cost. Message and data rates may apply. Your mobile carrier’s rates apply to messages you send and receive.
Opting out. Reply STOP, CANCEL, QUIT, END, UNSUBSCRIBE, STOPALL, OPTOUT, or REVOKE to any message to stop receiving texts. You will receive one confirmation message and no further messages after that. Reply START to resubscribe.

Carriers. Carriers are not liable for delayed or undelivered messages.
